1848, Monténégro. Vanko Winczlav, jeune médecin idéaliste, prend le parti de l'insurrection paysanne contre la tyrannie du prince-évêque. Vendu aux soldats par un paysan, il n'a d'autre choix que de partir. Dans sa fuite, il rencontre Veska, jeune Bulgare réduite en esclavage dans une auberge. Ensemble, ils embarquent pour le Nouveau Monde. Lors de la traversée, Vanko épouse Veska, qui n'a aucun papier d'identité. Arrivé sur le continent, il trouve un emploi d'infirmier, ne parvenant pas à faire valoir son diplôme de médecin.
